SMHS Athletics Package
All participating athletes are required to complete the Athletic Package. A physical examination may be required. The physical exam form must be completed by a licensed physician and valid (for one revolving year) through the end of the Track & Field season through end of May 2014. For clarification, please contact SMHS Health Office at (650) 558-2399.
If you've completed one for the Fall or Winter Sport for the current school year and is on file, you do not need to fill out another one. Prior to start of practice, please retrieve the Athletic Emergency (yellow) Card from SMHS Health Office, and bring to it Coach Riley during practice.
If Track & Field is the first after school program the athlete is participating in, please follow the links below to download (or pick one up at the Health Office), and complete. Submit entire package to the Health office located in the school's Building A. The Health Office will stamp its approval on the Athletic Emergency Card, return it to the athlete, to be hand-carried back to the Track & Field Head Coach, Ed Riley.
